By signing below, each listed business and undersigned owner, officer, or principal, individually and collectively (“Applicant”), certifies that all information and documents submitted are true, correct, and complete and authorizes Small Biz Heroes, LLC (“Broker”), its Marketplace Participants, and their respective representatives, successors, assigns, designees, brokers, funding intermediaries, and funding sources (“Recipients”) to obtain, share, transmit, and reproduce this application, its supporting documents, and any consumer, personal, business, investigative, credit, banking, or processing information needed to prequalify, underwrite, evaluate, or assist Applicant in obtaining commercial financing, including products with daily, weekly, or monthly payments or purchases of future receivables (“Transactions”). Applicant provides written instructions under the Fair Credit Reporting Act authorizing Broker and the Recipients to obtain information from TransUnion, Experian, Equifax, other credit bureaus, banks, creditors, financial institutions, and third parties, and authorizes those parties to release such information to Broker and the Recipients. If a Recipient requires another application format, Applicant appoints Broker as its limited agent to transfer and submit the information provided by Applicant without knowingly making material changes and authorizes Broker to reproduce and apply Applicant’s signature solely to those submissions. This authority remains effective until revoked in writing, but revocation will not affect actions taken before receipt. Applicant also authorizes Broker and the Recipients to contact Applicant by email, telephone, or text at any address or number provided or reasonably identified as belonging to Applicant, including wireless numbers and numbers on a Do Not Call registry, using automated dialing or similar technology regarding this application, future commercial financing opportunities, or other lawful marketing purposes. Applicant waives and releases claims against Broker, the Recipients, and information providers arising from the lawful requesting, receiving, sharing, reproducing, or releasing of information under this authorization, agrees to receive documents electronically, and acknowledges that Applicant’s electronic signature is legally binding and equivalent to a handwritten signature.
