By signing above, I agree to pay a fee for the services provided hereunder by Small Biz Heroes, LLC for securing, and or facilitating funding for the business listed. The Service Fee is assessed on the secured facility amount for credit lines and the actual funded contract amount for all other loans or advance products calculated in dollars at the % shown on the above schedule. The parties agree that the Service Fee represents the fair market value of the services provided by Small Biz Heroes, LLC and is a one time transaction to be processed after funding is completed and not a recurring fee. Products offered consist of term loans, lines of credit, equipment financing, commercial mortgages, invoice factoring, SBA loans, business cash advance & more. If funding is not completed for any reason, said fee will not be processed.
