I understand that by proceeding with SmallBizHeroes, LLC (SBH) I am authorizing SBH to represent me solely for the purpose of the application and financing process, whether written, electronic, verbal or otherwise. SBH does not have the authority to accept or decline any loan, line of credit, credit card, or other financing product on my behalf. I grant SBH the right to communicate with lenders and financing providers on my behalf verbally, electronically, or through email.

I agree that SBH has a funding consulting and placement fee equal to 9.9% of the total funding amount I accept and receive access to. The consulting fee is due to SBH within two (2) business days of receiving access to the funds. A late fee of $50.00 per day will accrue for each day the payment is late, beginning two (2) business days after receiving access to funds. SBH accepts payments by ACH, wire transfer, check by phone, or overnight delivery with tracking.

SBH charges NO UPFRONT FEE. The only time a fee is due to SBH is after I have been approved and received access to the funds.

I understand that SBH may submit or refer me to multiple lenders and financing providers in its database, each of which may conduct its own credit inquiry, underwriting, and approval process. I understand that disclosure of any third-party assistance may negatively impact my ability to obtain a loan, line of credit, credit card, or other financing product.

I am under no obligation to accept any loan, line of credit, credit card, or other financing product presented to me by SBH.

I agree to not utilize the same lenders or financing providers that SBH has applied to or recommended to me for a period of one hundred twenty (120) days. If I accept any loan, line of credit, credit card, or other financing product from any lender or financing provider recommended by or applied to by SBH, SBH’s funding contingency fee shall become immediately due and payable, together with any applicable late charges. I further authorize SBH to access and review my credit reports up to one hundred twenty (120) days following termination of services with SBH solely for the purpose of verifying whether the financing was obtained through any lender or financing provider recommended by SBH and whether the consulting fee has been paid.

Have you applied, or has anyone else applied on your behalf for any loan, line of credit, credit card or other financing product within the past sixty (60) days, or are there any banks, lenders or financing providers that you do not want SBH to contact on your behalf? If so, please list them below. These institutions will be exempt from this agreement.
